XXXXX
would benefit from Universal accommodations such as near rather than fine point
copying and/or notes provided depending on course content. Accommodations will
be applied by the subject teacher as needed. Oral testing will be used when
needed, to clarify responses.
SMART Goals:
XXXXX needs to remember to take his medication each
morning before coming to school and recording this on his phone calendar. There
are noticeable differences in his attitude and readiness for school when he has
not taken his medication. This will be evaluated after a month of monitoring
his attitude and readiness for school.
XXXXX will strive to have breakfast each day prior to
taking his medication. The medication he is taking inhibits appetite and
therefore he does not normally feel hungry during school hours. Eating
breakfast before going to school will assist with his energy levels and readiness
for school.
XXXXX should use the elevator when changing floors to aid
his fatigue. This will be reflected in his stamina throughout the day. Keeping
in mind that day to day his fatigue changes due to illness and other factors,
like extracurricular activities. This will be evaluated after a month to see if
perhaps it would be of benefit to have him move around less for class.
XXXXX needs to continually use assistive technology
for taking notes and submitting assignments. This will be reflected in more
assignments being passed in because of this usage. We will monitor it during
the first term and will evaluate and make changes accordingly.
